In enrolling at SUASL, participant understands that he/she attending the programs and using SUASL and the facilities does so at his/her own risk. SUASL and its owners, employees or agents, shall not be liable for any damage whatsoever arising from any personal injury or property loss sustained by participant with his/her family in or about any programs on the premises. Participants assume full responsibility for all injuries and damages which occur in or about any programs on the premises, He/She does hereby fully and forever release discharged hold harmless SUASL, all associated facilities and its owner, employees, and agents from any and all claims, demands, damages or rights of action, present or future resulting from any person's participation in any programs or use of the facility. In addition, he/she agree(s) to follow the rules of conduct and play set by SUASL. Failure to do so may result in suspension from participation. Consent: I do hereby authorize SUASL and its assigns to utilize any and all photographs, pictures or other likeness of me or anyone assigned guardianship to me, as they deem appropriate in its promotional materials or team films. --------------------------------------------------------------------- PLAYER CODE OF CONDUCT ACCEPTANCE SUASL is committed to providing a safe family sports and entertainment environment for all participants. To prevent violence and the potential for games to escalate out of control, it is extremely important that you as a player have also committed to making a difference in stopping and preventing violence. As a condition to be eligible in playing soccer for SUASL, I agree to honor the player Code of Conduct. I further understand that a violation of the Code of Conduct establishes grounds for suspension, fines and expulsion. 1. I shall not in foul and abusive language directly or indirectly at referees, players, teammates or spectators on or off the soccer field. 2. I will play competitively without unnecessary aggression, physical violence, and with no intent to injure another player. 3. When I am on the side lines, if a fight breaks out on the field, I will not enter the field and will encourage my team on the field to walk away. 4. If a fight breaks out between two players, my duty is to safely attempt to break up the fight and not join in, even if another player assaulted my teammate. 5. I will do whatever necessary to calm down my teammates and avoid escalation of violence. I will follow instructions from my team captain or manager. 6. I agree to support my team captain in enforcing the rules and player Code of Conduct established by the league. 7. I agree to walk away and not argue good or bad calls with the referee. 8. I agree to let the referee call the game and will refrain from shouting for calls. 9. I will use the bathrooms in and around the soccer field(s) and will not participate in any littering or indecent exposure in the parks where SUASL is operating. 10. I will not engage in drinking alcoholic beverages before and during my games. I will comply with all drinking rules established by the facility in use by SUASL and will acquire an alcohol permit where required. I will clean up the area used by my team. --------------------------------------------------------------------- TEAM MANAGER/CAPTAIN CODE OF CONDUCT ACCEPTANCE 1. As Captain and Team Manager, I accept full responsibility for my team's behavior on and off the field during our scheduled games on the soccer fields where SUASL operates. 2. I agree to pull off the field any one of my players whose behavior does not comply to SUASL's rules and player Code of Conduct. 3. I agree and will be responsible to host a team meeting where players receive behavior instructions and will read to them the Code of Conduct. I will collect from each player a signed copy of the Player Code of Conduct. Further I will distribute the league's rules to each player and will advise them to read them in its entirety. (All copies provided by the league upon request.) 4. I will enforce the side line behavior rules with relatives and friends of the team during our games, including never to enter the field of play specifically during an altercation between players. 5. I understand that the team depends on my leadership, exemplary conduct, and demonstration of respect for rules established by SUASL. I further agree that my leadership and playing privileges shall be revoked if I fail to behave according to the rules and Code of Conduct established by SUASL. 6. I understand that the league's privilege of operating in public soccer fields depends on the good behavior of all teams and players. I will carry that message to as many people as possible. 7. I will do whatever necessary to prevent and stop violence. --------------------------------------------------------------------- REFUND POLICY Player registration fee is non-refundable unless SUASL is not able place the team or player into the registered league program. Registration fees are accepted and processed on a first come first serve basis, therefore a spot is reserved for these players and teams and therefore a refund is not available. --------------------------------------------------------------------- By agreeing and submitting this registration application, I agree to abide by the laws and rules of the game of soccer as promulgated by the International Football Federation (FIFA), the United States Soccer Federation (USSF), and any properly affiliated league or team which may sanction a competition in which I choose to participate. I further agree that failure to abide by these laws and rules may result in the revocation of the right to play granted by the acceptance of this registration. In consideration for being allowed to participate in any USSF athletic/sports program, and related events and activities, the undersigned: Agrees that prior to participating, they will inspect the facilities and equipment to be used, and if they believe anything to be unsafe, they will immediately advise their coach or supervisor of such condition(s) and refuse to participate: 1. Acknowledge and fully understand that each participant will be engaging in activities that involve risk of serious injury, including permanent disability and death, and severe social and economic losses which might result not only from their own actions, inaction or negligence of others, the rules of play, or the condition of the premises or of any equipment used. Further, that there may be other risks not know to us or not reasonably foreseeable at this time. 2. Assume all the foregoing risks and accept personal responsibility for the damages following such injury, permanent disability or death. 3. Release, waive, discharge and covenant not to sue the USSF its affiliated clubs, their respective administrators, directors, agents, coaches, and other employees of the organization, other participants, sponsoring agencies, sponsors, advertisers, and if applicable owners and leasers of premises used to conduct the event, all of which are herein after referred to as releases, from demands, losses or damages on account of the injury, including death or damage to property, caused or alleged to be caused in whole or in part by the negligence of the release or otherwise.
